Aetna’s Medicare Standard Plan PPO has a monthly premium of $65.  For primary care visits, $15 copay is required when a Primary Care Physician, or PCP is selected and the plan is notified.  For specialist visits, $35 copay is required.  For impatient hospital care, there is $200 copay per day for days 1-5.  For diagnostic tests, x-rays, and lab services, the copay is between $35-$125. There are also pharmacy benefits.

PPO plans differ from HMO plans in that they offer the beneficiary the choice to choose from out of network health care professionals.  However, the deductible is usually smaller if an in-network doctor is chosen with a PPO plan.
