HumanaChoice H5826-063 PPO has a monthly premium of $0.  For primary care visits, $10 copay is required when a Primary Care Physician, or PCP is selected and the plan is notified.  For specialist visits, $30 copay is required.  For impatient hospital care, there is $550 copay per stay. For diagnostic tests, x-rays, and lab services, the copay is between $0-$95.  Pharmacy benefits are not included.
