exam, regular glasses, contact lenses or sunglasses. Every 24 months, the member can receive up to
$100 for frames and will only have pay $25 for most eyeglass lenses. Tonik can provide $80 for contacts every 2
years as well. There is a $25 deductible
for dentist visits and a $0 deductible for preventive and diagnostic care
regarding dental work. Rx covers generic
prescription drugs, and members pay $10 for 30-day supply from the pharmacy and
$20 for 90-day supply through mail order.
In-network Annual Out of Pocket (OOP) Maximum is $1,500. The Tonik 3000 plan is for the “part-time daredevil.” Within this plan, there is a $30 copay for
doctor visits, and a $3,000 deductible.
