All of these plans offer the traditional Health Maintenance Organization plans (HMOs) and Preferred Provider Organization plans (PPOs). HMOs are usually cost-effective in that the plan only requires payment on the monthly premium and deductibles that are paid at time of treatment.  However, PPOs are chosen by employers in times when employees have family doctors or specialists that they are comfortable with which are out of the plan’s network.  Employees are then able to choose PPO plans at a higher deductible.
